Financial Policy

You are responsible for all balances, co-payments, and deductibles, in full on the date of service. After the date of service, an additional interest charge will be incurred per month. If you are unable to pay, we offer healthcare financing through CareCredit.

Insurance coverage, deductible, and co-payment are only estimations, the actual amount is known after the claim is processed. You might be billed later for subsequent balances after claims are processed. If your insurance changes, is terminated, or payments are denied for any reason, you are responsible for all fees, costs, and balances not covered, paid, or remaining by your insurance. All balances are due at the time of service.

YYou are responsible to know if your insurance is active, in-network with our office, or has prior authorization requirements or any other type of benefit limitation for the services received and to pay all balances in full, whether insurance paid or not. For secondary insurance, you must provide the office with proper information and timely primary insurance EOB. If not done so, your secondary will not be billed, and you are responsible to pay the balance and then file the secondary claim for reimbursement, on your own.
